By entering your contact information above, you will be entered into the giveaway and agree to receive marketing communications from CIVANA Wellness Resort & Spa and Spiritual Gangster. Only one form entry per person; bonus entries apply to social tags only.

Terms and Conditions:
Guests must be 18 years or older to book a room at CIVANA. The minimum age for program guests is 14 years old. Children under 14 are not permitted. Persons aged 14 to 17 may stay with an accompanying adult over 18 in the same room. The minimum age for access to the spa is 16 years old.

This giveaway includes: One (1) two-night stay for two (2) guests at CIVANA Wellness Resort & Spa in Carefree, Arizona; one (1) 60-minute spa treatment per guest (two total treatments); $100 resort credit; one (1) CIVANA wellness journal and water bottle per guest; one (1) $500 gift card from Spiritual Gangster; and roundtrip ground transportation between CIVANA and Phoenix Sky Harbor International Airport, if needed (collectively, the “Experience”).

The giveaway prize is non-transferable, non-refundable, and may not be exchanged for cash or resort credit. Any supplemental purchases not listed here (e.g., additional spa treatments, alcohol, retail, or excursions) are the responsibility of the guest. CIVANA reserves the right to substitute elements of the prize package in the event of scheduling or operational changes. Reservations are subject to availability and blackout dates may apply. The winner must redeem their stay within six (6) months of the giveaway close date, excluding major holidays and peak season. Airfare and other travel costs to and from Phoenix Sky Harbor Airport are not included and are the responsibility of the winner. Some experiences may require a signed liability waiver.

Check-in is at 4:00 PM MST; check-out is at 11:00 AM MST. Guests must provide a valid credit card upon reservation and check-in to cover incidentals. If the winner chooses to extend their stay, additional nights will be at their own expense. The spa treatment credit is applicable to services up to $215 (per person). Service charge is included. Retail purchases are not included. Treatment credit is non-cumulative, non-transferable, and limited to one per guest. Spa appointments should be reserved in advance. Food and beverage service charge is included. Alcohol, zero-proof cocktails, packaged snacks, and retail food/beverage items are not included.

No substitutions will be made outside the package as described. Entry into the giveaway constitutes acceptance of these terms and conditions.
